OUR SERVICES
At Country Community Care, we recognise that each participant has a unique set of limitations and needs, so we tailor our programmes to meet their needs as well as those of their family. Talk to us about your requirements and plan, and we’ll assist you receive NDIA approval. We are permitted to offer the following NDIS support services to individuals with disabilities as an authorised NDIS provider.
Accommodation/Tenancy
Our accommodation services include:
- short and medium-term housing
- respite care
- supported independent living
- specialist disability housing (SDA)
Assist Access/Maintain Employ
Individuals and groups receiving workplace support might use professional networks to find and contact employers. The Assist Access/Maintain Employee programme assists employees in keeping their jobs after they have been hired.
Participants who obtained a job before enrolling in the NDIS are also eligible for assistance.

Assist-Life Stage, Transition
Support participants so that they are more aware of the aim of the provided supports and are better able to participate in their communities by increasing their connections to larger support networks.
Country Community Care will assist a participant in understanding the plan by managing continuous support and responding to queries as they occur.Country Community Care’s short-term and long-term support focuses on staying at home, participating in the community, and coordinating your care.
Assist-Personal Activities
We support your choice to live the life you want by assisting you with daily living. We offer flexible and tailored support based on your preferences, needs, objectives, and outcomes. Our skilled and courteous professional nursing agency team is here to assist you manage and achieve your daily routine.
Home-based assistance are typically available, however community-based supports are provided based on the individual’s choices. Our daily personal activities help is tailored to our clients’ unique needs and requirements. Please feel free to contact us if you require assistance.
Assistance Travel/Transport
In addition to transportation, we help our participants to receive the proper tools and assistance they need to travel independently. We also teach customers how to use public transit properly, making their trips more convenient and comfortable. We cover everything for you.

Innovative Community Paticipation

Development-Life Skills
Country Community Care will help you develop a talent that is unique to you. We help you live independently while also increasing your communication abilities. Our goal is to help you, your family, or your carer navigate all of the challenges you experience on a daily basis and provide you with coping methods.
As persons who have worked within the disability and community sectors for many years, we’ve noticed and acknowledged the difficulty of addressing ordinary jobs and activities by people with impairments.
Household Tassk
Our NDIS provider for household activities will assist you with any everyday living issues you may be facing. We are the top NDIS service provider, and we will assist you with household task that have been a difficulty for you. NDIS, you can entrust us with any domestic duty.
A clean and well-organized house is necessary for good health and happiness. The NDIS household jobs category includes domestic chores and household activities.

Participate Community
Equal access to the following services is critical for successful community engagement and social inclusion support:

Spec Support Employment
Participants who held a job before to enrolling in the NDIS are also eligible for assistance. Some types of assistance are subject to Commonwealth Quality and Safeguards (for example, National Standards for Disability Services certification received through the Human Services Scheme); however, it rests entirely on the jurisdiction.
At Supported Employment, we provide the following forms of assistance:
- Pre- and post-employment job assistance
- Assistance with group job preparation
- Helps school leavers find work, allowing for a smooth transition from school to the labour market.
- Supporting NDIS participants' further education and training

Assist/Maintain Education
A person’s capacity to find – and keep – meaningful and productive work is heavily influenced by the quality of his or her access to higher education, vocational training, and skill development. Job support provided by the government, non-profit organisations, or educational institutions can bridge the training and employment gap that hinders those who want to work from earning a living wage.
